ICC Patient Access is a support service for physicians and their patients that provides effective tools for continuity of care and for enhanced connection between all providers, and patients in and outside the home.

ICC Patient Access is connected to a secure data repository, where critical information has been entered by the primary physician to be retrieved by other clinicians in case of emergency. This obviously requires the patient’s or his family’s consent. It also works as a portal whereby patients can be linked with their caregivers. Each patient has his or her own individual patient record, accessible with a password, from anywhere at any time. ICC Patient Access makes available the salient facts relating to the risk factors involved whose profile information can then be shared with physicians in case of emergency or for consultation with other specialists.
